Qualified Plan Beneficiary Form Quick Test

If you can answer "Yes" to any of the following questions, you most likely are in need of our consulting services:*

After reviewing your current IRA,  401-k, or 403-b (or any other qualified funds plan) beneficiary form(s):

1. Have you listed your minor child or estate as your primary beneficiary?
2. Have you failed to list your spouse as primary beneficiary?
3. Have you listed a former spouse that is no longer applicable?
4. Have you listed more than one adult child as primary or contingent beneficiaries but did not include special language requiring the administrator to divide the accounts equally so that each child has a "separate" account?
5. Have you listed a Living Trust as beneficiary prior to January 11th, 2001? If so, does that trust meet the minimum IRS requirements to receive these funds and "see through" to the designated beneficiary?
6. Have you created a custom qualified plan beneficiary form with an advisor, but it was before January 11th, 2001, and had not been changed since?
7. Have you named a non-relative friend, neighbor, or charity as beneficiary before January 11th, 2001?
8. Are you unable to find a copy or obtain a copy of your qualified plan beneficiary form(s)?
9. Have your current advisors told you, "The way you have it is O.K."?  Or told you it wasn't important?
10. Have things changed in your life since you filled out the form? Such as something as simple as moving?  Or, is a change needed on who gets what and when?
